Italics and quotation marks

When a character says something with a lot of emphasis (using italics), and they say the whole thing with emphasis, even if it's just one word (ex, "WHAT?"), do you put the quotation marks in italics? And what about the other punctuation?
Ex, ( mean italics)
1. "What am I going to do with you?"
2. "What am I going to do with you?"
3. "What am I going to do with you?"

Which one is correct? I always use number 2, but Stephenie Meyer uses 3. (quote from Twilight)

ANSWER

Most editors advise rarely using italics for emphasis in dialogue, so do so sparingly.

However, if you have an entire line in italics, include the punctuation and quotation marks, unless the passage is quoted:

"I was with him when he approached her and all she said was What?" Here the quotation marks belong with the speaker, not the quoted word, so they would be unitalicized.

If a single word is in italics, only the word is italicized:

"You did what?"
